Get help understanding any changes to your medicines after a hospital stay, so you can take them safely at home
Coming home from hospital can be unsettling, especially if your medicines have changed while you were away. The Discharge Medicines Review is a free service, funded by NHS Wales as part of the Community Pharmacy Wales Good Health – Iechyd Da initiative. Our pharmacist goes through your medicines with you to make sure the changes made in hospital are carried through correctly at home.
Who it is for
You can have a review if you have been discharged from a care setting with a Discharge Advice Letter and one or more of the following applies:
- Your medicines were changed during your stay
- You take four or more medicines
- You need help remembering when to take your medicines, and a compliance aid might suit you
If you would rather not take part yourself, a carer or representative can do it on your behalf. Not sure whether you were given a Discharge Advice Letter? Just ask a member of our team.
What happens
The review takes place in two short conversations. We check you have everything you need, stop anything no longer required, sort out any discrepancies, and answer your questions about new medicines. With your consent, we can share information with your GP where needed.
It helps to bring your Discharge Advice Letter, your first prescription since leaving hospital, and a note of any questions.
